Why do I need to provide a ProForma IRS Form 990,
if the IRS only requires me to submit a 990EZ/990PF (or comparable form)?
The IRS Form 990EZ (or comparable form) does not include all of the financial information required by the CFC for the review of the organization’s eligibility.

What pages of the IRS Form 990 create a ProForma 990?
a. Page 1 (Part I, Summary and Part II, and Signature Block)
b. Pages 7-8 (Part VII, Compensation Sections A & B)
c. Page 9 (Part VIII, Statement of Revenues)
d. Page 10 (Part IX, Statement of Functional Expenses)
e. Page 11 (Part XI, Financial Statements and Report)

How do I calculate my AFR?
Click here for AFR calculator. Or, Add the amount listed in Part IX, Line 25, Column C to the amount in Line 25, Column D, and divide the sum by Part VIII, Line 12, Column A.

What is a Bona Fide Chapter Letter, and how do I know if I need to submit one?
See CFC Memorandum 2009-8 for a detailed explanation.

How do I know if I'm a federation, or if I qualify for federation status?
If your organization has 15 or more qualifying member agencies your organization can apply for federation status.

Why do I need an Independent Audit and not an Internal Audit?
An audit prepared by an independent CPA in accordance with Generally Accepted Auditing Standards (GAAS) is required.  An internal audit or review does not meet these requirements.

I've submitted an IRS Determination Letter with my application, why am I being asked for an updated affirmation letter?
The IRS may revoke an organization’s tax-exempt status at any time for a variety of reasons. While the IRS does not reissue Determination Letters, they do issue Affirmation Letters. The CFC verifies that current status of each participating charity on an annual basis. To assist in this process we request each organization verify their status with the IRS and request an Affirmation Letter, particularly if the IRS Determination Letter is over 10 years old, does not list the current business name, and/or organization address. If the CFC is unable to confirm the current status of an organization, that organization must contact the IRS Tax-Exempt Division to correct the issue and provide adequate documentation to the CFC.

Applications

Application Period
The 2012 application period to participate in the 2012 charity giving campaign for the federal workforce opens Monday, February 1, 2012 and will close Monday, MARCH 1, 2012 (all entries must be postmarked by, or prior to this date). Late submissions will not be accepted!

All area non-profits are encouraged to apply. Local federal volunteers review applications for eligibility. General requirements include recognition by the Internal Revenue Service as a tax-exempt organization under 26 USC 501(c)(3). See application for more details.

Green Initiative
When submitting an application, please consider the following steps to reduce the CFC Application carbon footprint:

1. Provide applications and attachments on double sided copies and print-outs whenever possible, particularly FInancial Audits (Attachment C) and 990's (Attachment D).
2. Submit applications and attachments unbound and without 3-ring binders, binding combs/spirals, paper/plastic folders, plastic binders, plastic sheet protectors, plastic dividers, etc. Binder clips, paperclips or stapled documents are preferred.
3. Paper can also be saved by writing the attachment name (ie."Attachment - C") on the top right corner of the first page of the attachment, rather than printing a separate attachment cover sheet.
4. Paper can also be saved by submitting ONE Attachment E.
5. Do not include a cover letter.
6. Submit only the required attachments detailed in the application, label and group them appropriately.
7. Submit the application in only ONE envelope, or box.

Tax Exemption Status
Check the IRS Exempt Master Listing to ensure your agency is in good standing:
www.irs.gov/taxstats/charitablestats/article/0,,id=97186,00.html

If your agency is not listed, or cannot be found, on this master listing you will need to contact the IRS Tax-Exempt Division at 877.829.5500 and request an "Affirmation Letter" (dated January 1, or later, of the application year) and submit the letter with your application, or to the campaign. Bona-Fide chapters and affiliate organizations of national organizations not found on the master list, need to follow documentation requirements as described in the 2010 CFC application.
